10 Free OSHA Forklift Practice Questions
Q1. The stability of a forklift is based on a geometric shape known as the: Show answer
✓ Correct Answer: Stability triangle
Forklift stability is based on a three-point suspension system, creating a stability triangle formed by the two front wheels and the center of the rear steer axle.
Q2. A 'combined Center of Gravity' refers to the combination of:Show answer
✓ Correct Answer: The forklift and the load
The effective CG that determines stability is the combination of the truck's mass and the load's mass.
Q3. If the hydraulic system is leaking oil on the floor, the operator should:Show answer
✓ Correct Answer: Remove the truck from service
Leaks create slip hazards and indicate a failure in the lifting or braking systems. The truck is unsafe to operate.
Q4. If a trailer is not coupled to a tractor (semi-truck), what extra precaution is needed?Show answer
✓ Correct Answer: The trailer nose must be supported by a fixed jack (trailer stand) to prevent upending
A heavy forklift entering an uncoupled trailer can cause the trailer to tip forward (nose dive). Fixed jacks support the nose.
Q5. High-visibility vests are:Show answer
✓ Correct Answer: Recommended for all personnel in mixed-use areas to increase visibility
Hi-viz clothing helps operators spot pedestrians sooner.
Q6. When traversing a ramp, the forklift's center of gravity shifts:Show answer
✓ Correct Answer: Downhill
Gravity pulls the center of mass toward the downhill side relative to the wheelbase, reducing the stability margin.
Q7. Trailer creep (dock walk) occurs when:Show answer
✓ Correct Answer: The trailer moves away from the dock due to the motion of the forklift entering/exiting
The momentum of the forklift entering can push the trailer forward, creating a gap that the forklift can fall through. Wheel chocks prevent this.
Q8. If an operator has not operated a specific type of truck before:Show answer
✓ Correct Answer: They must receive training/evaluation on that specific type
Certification is type-specific (e.g., a sit-down operator is not certified for a stand-up reach truck without specific training).
Q9. What is the primary cause of forklift tip-forward (longitudinal tip-over)?Show answer
✓ Correct Answer: Overloading or carrying loads that shift the CG past the front axle tip line
Tip-forward occurs when the combined CG moves beyond the front axle tip line — caused by overloading, excessive load center distance, braking while loaded, or lifting with the load too far forward.
Q10. What is the proper procedure for inspecting forklift lift chains?Show answer
✓ Correct Answer: Visually inspect for rust, wear, cracks, and stretched/elongated links; check lubrication; verify equal tension between left and right chains
Lift chain inspection includes: checking for rust, corrosion, worn or cracked links, elongation (stretch), and adequate lubrication. Both chains must have equal tension — unequal tension can cause tilting and uncontrolled load movement.
What Does the OSHA Forklift Certification Cover?
OSHA forklift (powered industrial truck) certification is employer-based rather than a single national exam. Under 29 CFR 1910.178 the employer must provide formal instruction, a written knowledge test, and a hands-on driving evaluation, then certify the operator for that specific truck type and workplace. The written portion is usually 20–40 multiple-choice questions with a 70–80% passing threshold set by the employer or training provider, and it covers load stability, the stability triangle, pre-shift inspection, refueling, and pedestrian safety. Certification is specific to the truck class and workplace, and operators must be re-evaluated at least every three years.
How Hard Is the OSHA Forklift Certification?
OSHA forklift certification is one of the easier safety credentials, with pass rates typically above 90%, because it is employer-based training rather than a standardized national exam. The written portion is straightforward, but operators must also pass a hands-on driving evaluation and be re-evaluated at least every three years or after any accident or unsafe operation. There is no single national forklift exam — each employer tests operators, and an operator certified on one truck type is not automatically qualified on another.
How to Study for the OSHA Forklift Certification Test
- 1.Master the stability triangle — understand how load center, load weight, and mast tilt shift the truck's center of gravity. Capacity and tip-over questions dominate the written test.
- 2.Memorize the pre-operation inspection — know the pre-shift inspection checklist cold; it is a high-yield topic on the written portion.
- 3.Learn the safe-operation rules — sound the horn at intersections, no riders, travel with forks low, and look in the direction of travel.
- 4.Practice for the hands-on evaluation — you must also pass a driving evaluation. Practice smooth control of travel, steering, and load handling.
- 5.Know your truck class and renewal — certification is specific to the truck class (sit-down counterbalance, reach truck, order picker, and so on) and workplace, and requires re-evaluation at least every three years.
