About the Phlebotomy Certification Exam

Phlebotomy certification is offered by two major organizations: NHA (National Healthcareer Association) and ASCP (American Society for Clinical Pathology). Both are nationally recognized and equivalent in value. Certification demonstrates competency in blood collection, specimen handling, and infection control.

NHA CPT exam: 120 questions (100 scored, 20 experimental), 2 hours, scaled passing score of 390/500. ASCP PBT: 80 questions, 2 hours, scaled passing score of 400. Both test the same core topics: venipuncture technique, order of draw (CLSI GP41), tube additives and inversions, specimen collection and handling, infection control, and patient safety.

The order of draw and tube identification questions are weighted heavily — candidates must know 30+ tube types by color, additive, inversion count, and intended use. Pre-analytical errors (improper collection, handling, or processing) cause incorrect lab results, so these questions are critical. Certification is valid 3 years and requires recertification through retesting or continuing education.

Order of Draw — Quick Reference

The single most-tested topic on the NHA CPT and ASCP PBT. Drawing tubes out of sequence lets additives carry over between tubes and corrupts results. Memorize this CLSI sequence cold.

#TubeAdditiveCommon TestsInversions
1Blood culture (yellow / bottles)SPS / sterile brothBlood cultures8–10
2Light blueSodium citrateCoagulation (PT, PTT, INR)3–4
3Red / gold (SST)Clot activator / gelSerum chemistry, serology5
4GreenHeparin (lithium/sodium)Plasma / STAT chemistry8–10
5LavenderEDTAHematology (CBC), blood bank8–10
6GraySodium fluoride / oxalateGlucose, lactate8–10

Why the order matters

Each tube's additive can contaminate the next. EDTA (lavender) carryover into a chemistry tube falsely raises potassium and lowers calcium; heparin (green) carryover skews coagulation results. Blood cultures go first to keep the draw sterile, and clot/serum tubes precede anticoagulant tubes. Inversion counts mix the additive evenly — too few clots the specimen, too many causes hemolysis.

CLSI GP41 standard. Inversion counts are typical manufacturer ranges; always follow your tube maker's insert.

Frequently Asked Questions

Should I take the NHA CPT or ASCP PBT exam?

Both certifications are nationally recognized and equivalent in value. NHA CPT is more common in clinical labs, ASCP PBT in hospital labs. NHA CPT has 120 questions (100 scored + 20 pretest) over 2 hours. ASCP PBT has 80 questions over 2 hours. Both test the same core competencies — order of draw, tube identification, venipuncture technique, and specimen handling. VoltExam prepares you for either exam.

How many questions are on the phlebotomy exams?

NHA CPT: 120 questions (100 scored, 20 experimental), 2 hours. ASCP PBT: 80 questions, 2 hours. Both are scored on a scaled range, not a raw percentage — NHA CPT passes at a scaled 390 (range 200–500); ASCP PBT at a scaled 400. Question difficulty is moderate, but the time pressure is real. VoltExam simulates both exam formats.

What's the hardest part of the phlebotomy exam?

Order of draw and tube identification. Candidates must know every tube color, additive, number of inversions, and intended use — cold. We're talking 30+ different tube types. If you can't identify SST vs PST vs lithium heparin just by color, you will miss multiple questions. VoltExam drills tube colors and additives relentlessly.

How long should I study for the phlebotomy certification exam?

Most phlebotomy students study 40–80 hours during their certificate program, then 15–25 hours of focused prep before the exam. With VoltExam, many candidates study 2–3 hours per week for 6–8 weeks. Focus on tube identification and CLSI order of draw — these are weighted heavily.

Can I work as a phlebotomist while waiting for my exam results?

It depends on your employer and state. Some employers allow you to work under supervision while awaiting results. Others require certification first. The most common practice is to work as a phlebotomy technician (unlicensed) while awaiting your certification exam. Check with your employer.

How long is the phlebotomy certification valid?

NHA CPT and ASCP PBT certifications are typically valid for 3 years. Some employers require annual continuing education. After 3 years, you renew by retaking the exam (recertification exam is usually the same or easier than initial exam). Maintaining current knowledge is essential — techniques and equipment change.
